Report Concerns

There are two ways to let the College know if you have a concern about the care or services provided by a pharmacist, pharmacy technician or pharmacy in Ontario: report information or file a complaint.

Option 1: Report Information

If you report information:

  • The College will assess your concerns and take appropriate action.
  • You may be contacted and asked to provide additional information.
  • Generally, you will not be engaged in the process and will not be notified of the outcome.
  • Any information that you provide including your personal information (such as your name, telephone number and email address) and your personal health information (such as your pharmacy and medical records) may be provided to the registrant during the investigation. The registrant is required to keep this information confidential.

Option 2: File a Complaint

If you file a complaint, you will be involved in the process and notified of the outcome. It also means:

  • You cannot make an anonymous complaint. A copy of your complaint including your name will be provided to the registrant. Any additional information that you provide such as your personal information (such as your telephone number and email address) and your personal health information (such as your pharmacy and medical records) may be shared with the registrant. If you are filing a complaint on behalf of someone else, their personal information and personal health information may also be provided to the registrant. The registrant is required to keep this information confidential.   • You will receive a copy of the complaint decision and reasons.
  • You can request a review of the decision by the Health Professions Appeal and Review Board.
